Participant understands that changes to class time and/or class level during the registered semester are not permitted without prior written notice to Rebecca Chess Club . Further, if participant chooses to make Changes, Participant must notify Rebecca Chess Club, in writing , at least ten (10) days prior to the start of Participant’s first lesson (Deadline). The granting of such changes is at the sole discretion of Rebecca Chess Club. If such changes are granted after the Deadline, a $50 processing fee per occurrence shall be.

Participant understands that class enrollment requires participant’s commitment for the whole semester. PLEASE NOTE:❗ TUITION IS NON-REFUNDABLE ❗ If Participant drops out of any class before the semester ends, no credit towards a future semester will be issued. Participant further understands and agrees that Participant, withdraws from any class, Participant must notify Rebecca Chess Club, in writing, in advance, and Participant shall not be permitted to attend any remaining classes for any reason.

Copyright Agreement: Rebecca Chess Club 对所有的文件和影像资料拥有版权。所有的文件和视频所有权属于Rebecca Chess Club，任何非法传播将会追究法律责任。 Rebecca Chess Club is the sole legal owner of all class recordings and any other class-related materials, including, but not limited to, weekly class handouts and game review videos. These materials are copyrighted and shall not be duplicated, disseminated, or appropriated without prior written permission from Rebecca Chess Club. Any illegal downloading or distribution of Rebecca Chess Club’s copyrighted materials may result in criminal prosecution and/or in a lawsuit in civil court, seeking damages, and all available remedies, including attorney’s fees and court costs.

补课政策：Participant understands and accepts that if Participant child misses a class, there will not be a live makeup Zoom lesson. If Participant misses a class, it can be made up only by watching the class recording. The validity period of the video access permission is within three months after the end of the semester. 如果学员因故缺课，将不会提供实时的Zoom 补课。缺课的课程只能通过观看课堂录播视频进行补课。视频观看权限的有效期为该学期结束后的三个月内。

RCC反作弊条款： RCC非常注重学生的素质和体育精神的培养，我们的比赛严禁作弊，同时我们也不希望任何非正常行为影响对局的质量。任何作弊（软件，手机，电脑，父母兄弟姐妹提示等）是对自己和他人的不负责任。 如果比赛中发现作弊证据确凿，第一次将严重警告。第二次将永远从本俱乐部除名并且不予退还学费。 对于 chess.com 因为Fair Play Policy 封号的情况，在学生没有申诉成功前，所有的内部赛和班级赛的成绩都将清零。请仔细阅读chess.com 的规定：https://www.chess.com/legal/fair-play RCC有权判定对局的真实性和合理性，并对高度疑似作弊的对局有判定权。 Rebecca Chess Club’s Fair Play Policy/Anti-Cheating Stance: 1). All of Participant’s moves must be his/her own. 2). Do not cheat in any way. 3). Do not get help from any other person, including parents, siblings, friends, coaches or another player. 4). Do not use chess engines, software of any kind, bots, plugins or any tools that analyze positions during play. Rebecca Chess Club uses chess.com to conduct club’s after-class games and internal tournaments. If Participant is banned by Chess.com due to a fair play policy violation, Participant shall receive one (1) warning. However, if Participant violates Rebecca Chess Club’s fair play policies more than once, Participant shall be banned from the Club and no refund shall be issued. Rebecca Chess Club is committed to creating a clean and fair environment for all its students. Participant accepts and agrees to adhere to the above-mentioned fair play policy.

This registration form is written in the Chinese and English languages and both languages shall have equal validity. If there is any conflict or inconsistency between the Chinese version and the English version, the English shall be the governing and prevailing version.

Release and Waiver of Liability PLEASE READ CAREFULLY! THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS! This Release and Waiver of Liability (the “Release”) executed on the day signed by the participants who is signing this agreement (All the “Participants”) in favor of Rebecca Chess Club, owned and operated by ZHAO & ASSOCIATES, LLC, a limited liability company registered at 4702 Payton Chase Ln, Katy, TX, 77494 its affiliated clubs, their respective administrators, directors, agents, and other employees and volunteers of the organization, other members/participants, sponsoring/hosting agencies/universities, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as “releases,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses, or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise. The Participant hereby freely, voluntarily, and without duress executes this release under the following terms: Release and Waiver: Participant does hereby release and forever discharge and hold harmless Rebecca Chess Club and ZHAO & ASSOCIATES LLC and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Participant’s Activities with Rebecca Chess Club and ZHAO & ASSOCIATES LLC. Participant understands that this Release discharges Rebecca Chess Club and ZHAO & ASSOCIATES LLC from any liability or claim that the Participant may have against Rebecca Chess Club and ZHAO & ASSOCIATES LLC with respect to any bodily injury, personal injury, illness, death, or property damage that may result from Participant’s Activities with Rebecca Chess Club and ZHAO & ASSOCIATES LLC, whether caused by the negligence of Rebecca Chess Club and ZHAO & ASSOCIATES LLC or its officers, directors, employees, or agents or otherwise. Participant also understands that Rebecca Chess Club and ZHAO & ASSOCIATES LLC does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness. Medical Treatment: Participant does hereby release and forever discharge Rebecca Chess Club and ZHAO & ASSOCIATES LLC from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Participant’s Activities with Rebecca Chess Club and ZHAO & ASSOCIATES LLC. Assumption of the Risk: The Participant acknowledges and fully understands that each participant will be engaging in Activities that may be hazardous, involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time. Participant hereby expressly and specifically assumes the risk of injury or harm in the Activities and releases Rebecca Chess Club and ZHAO & ASSOCIATES LLC from all liability for injury, illness, death, or property damage resulting from the Activities. Insurance: The Participant understands that, except as otherwise agreed to by Rebecca Chess Club and ZHAO & ASSOCIATES LLC in writing, Rebecca Chess Club and ZHAO & ASSOCIATES LLC does not carry or maintain health, medical, or disability insurance coverage for any Participant. Each Participant is expected and encouraged to obtain his or her own medical or health insurance coverage. Photographic Release: Participant does hereby grant and convey unto Rebecca Chess Club and ZHAO & ASSOCIATES LLC all right, title, and interest in any and all photographic images and video or audio recordings made by or shared with Rebecca Chess Club and ZHAO & ASSOCIATES LLC during the Participant’s Activities with Rebecca Chess Club and ZHAO & ASSOCIATES LLC and during the participant’s activities in other chess events, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. Parent/Guardian Waiver for Minors (Under 18 years old): The Participant acknowledges and represents as undersigned parent and natural guardian or legal guardian acting in such capacity and agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian. Other: Participant expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the State of IL and that this Release shall be governed by and interpreted in accordance with the laws of the State of IL. Participant agrees that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.

Media Consent Form and Release I hereby consent to have the participant's child photographed, digitally recorded, video taped, audio taped and/or interviewed by Zhao & Associates LLC (DBA: "Rebecca Chess Club") or the news media when the school is in session, either in person or hosted online, or hosted remotely, or when my child is under the supervision of Zhao & Associates LLC (DBA: "Rebecca Chess Club"). Further, I consent for these photos, digital recordings, video tapes, audio tapes and/or interviews to be shared with third parties who have received written approval from Zhao & Associates LLC (DBA: "Rebecca Chess Club"). I understand in the course of the above described activities that Zhao & Associates LLC (DBA: "Rebecca Chess Club") might like to celebrate my child’s accomplishments and work. Therefore, I further consent for Zhao & Associates LLC (DBA: "Rebecca Chess Club") release of information on my child’s name, online account, academic/non-academic awards and information concerning my child’s participation in Zhao & Associates LLC (DBA: "Rebecca Chess Club") sponsored activities, organizations and athletics. I also consent to Zhao & Associates LLC (DBA: "Rebecca Chess Club")’s use of my child’s name, photograph or likeness, voice or creative work(s) on the Internet or on a CD or any other electronic/digital media or print media electronic which may include honorary banners/signs displayed in, near, or around the school building or community. As the child’s parent or legal guardian, I agree to release, indemnify and hold harmless Zhao & Associates LLC (DBA: "Rebecca Chess Club"), its members, trustees, agents, officers, contractors, volunteers and employees from and against any and all claims, demands, actions, complaints, suits or other forms of liability that shall arise out of or by reason of, or be caused by the use of my child’s name, photograph or likeness, voice or creative work(s), on television, radio or motion pictures, or on the Internet, or on a CD, or any other electronic/digital media or print media or in connection with my child’s participation in virtual school events and/or celebratory activities. It is further understood and I do agree that no monies or other consideration in any form, including reimbursement for any expenses incurred by me or my child, will become due to me, my child, our heirs, agents, or assigns at any time because of my child’s participation in any of the above activities or the above-described use of my child’s name, photograph or likeness, voice or creative work(s). I understand that I may cancel this consent by providing written notice to the principal. I also understand that my consent is valid in perpetuity.
